My wife's Mid-2010 Mac Mini is acting very strangely. The other day I went to upgrade it to Mountain Lion and during the initial install the disk failed the check. I tried to repair the disk but that failed. Can't recall the specific error. This was all done within the Mountain Lion install process (which only worked once). After I restarted and selected the recovery partition to try and repair the drive that way, the Apple screen showed with the loading icon, and goes nowhere.

I can select the USB drive as the boot disk, but then all that happens is that the grey Apple screen with the loading icon appears..and then just sits there, doing nothing. For hours. I tried this using a DVD with Mountain Lion as well.

I have reset the SMC and the PRAM/NVRAM.
Is it possible that a borked hard drive would prevent the OS being loaded from a USB drive? ie: the files can't be written to a temporary location on the drive in order to boot?

A prohibitory symbol, which looks like a circle with a line or slash through it, means that your startup disk contains a Mac operating system, but it's not a version or build of macOS that your Mac can use.
Create Macos Bootable Usb
- Press and hold the power button on your Mac for up to 10 seconds, until your Mac turns off.
- Turn on your Mac and immediately press and hold both Command (⌘) and R to start up from macOS Recovery.
- While in macOS Recovery, use Disk Utility to repair your startup disk.
- If Disk Utility found no errors or repaired all errors, reinstall macOS.
